CHANGING TIDES: INNOVATIONS IN PALLIATIVE CARE
KEYNOTE SPEAKER: Lucy Kalanithi, MD, FACP, a Yale-educated internal medicine physician and faculty member at the Stanford School of Medicine, is the widow of the late Dr. Paul Kalanithi, author of the #1 New York Times bestselling memoir, When Breath Becomes Air, for which she wrote the epilogue. Surveying her career as a medical professional and drawing from her personal experience, standing alongside her husband during his life, diagnosis, treatment, and death, Dr. Kalanithi has special interests in healthcare value, meaning in medicine, patient-centered care, and end-of-life care.

SDCCC is re-convening our annual meeting, "The Road Ahead for Serious Illness Care in San Diego,” and we need your help! Join us as we launch a major new educational initiative around community awareness of palliative care and advance care planning. Help us as we plan a new campaign of activities and events, and work with us to make an impact in your community, organization, or patient population. Our initiative is now supported by a generous gift of funding, and we are very excited to get to work. Last year, we identified gaps and opportunities for action; this year, we’ll make concrete plans and get things going.

Broadly approaching the concept of quality, participants can look forward to keynote, plenary, concurrent sessions, and posters that address quality from every angle: curriculum planning, learning application, outcomes and process improvement, research, decreasing health disparities, and more.
Each of our lives is unique. However, there is one thing we all have in common: we do not know the end of our own stories. For many, the topic of death has been off- limits. The time has come to ask ourselves and the medical community the following questions:
• What can we do to help create a fitting end to our story?
• How do we best extend compassionate care to those reaching the completion of their lives?
• Can we create a supportive environment where dignity, comfort and meaning may be found in every lived moment?
Our nation is experiencing a moment of inspired and remarkable cultural change in how we think about and approach the end-of-life. Join us as doctors Shoshana Ungerleider, BJ Miller and Michael Fratkin discuss what it means to live well until the end, as we explore the power of our relationship with death and dying. Learn from their unique perspectives, filled with wisdom and knowledge gained from their own lived experiences.
THE EVENING WILL BEGIN WITH A RECEPTION FEATURING A PHOTO EXHIBIT FROM THE FOUNDATION'S "PORTRAITS OF LOVE" PROGRAM. LIGHT SNACKS AND BEVERAGES WILL BE SERVED.
At the conclusion of this educational activity, participants will be able to
discuss the importance of advance care planning in the healthcare setting,
review the principles of palliative care medicine,
describe the principles of evidence‐based medical decision making.
A two-day program designed to help healthcare providers develop the skills necessary to have quality conversations about medical interventions at the end of life.
